Texas Healthy Communities (TXHC) Programs helps communities assess their existing environments and use this assessment to implement system changes, improve upon policies, and promote public health practices that reduce risk factors for chronic diseases. Texas communities involved in this program are assessed on 8 indicators: physical activity, nutrition, breastfeeding, schools, work sites, tobacco, cardiac and stroke response, and healthcare quality.

- Healthy food options are accessible and promoted
- Mother friendly work-sites, birthing facilities, and hospitals are in place in the community
- Comprehensive tobacco control program is in place and includes a 100% smoke free city smoking ordinance
- Health care sites in the community promote primary and secondary prevention of cardiovascular disease and stroke.
